The assessment allows you to demonstrate an understanding of the complexities of multimorbidity for a person with a disability and chronic condition. This assessment will develop and expand on your ability to identify factors which inhibit independence and diminish quality of life. You will need to consider issues that inform behaviour and advocate for practical strategies to improve independence and quality of life. Due Date Wednesday 10th April 2024. Time Due 14:00. Weighting 50%. Length E-poster with a 10-minute (+/- 10%) narration (no video required). Rubric Available via the ‘Rubrics’ tab of the NRSG372 Canvas unit. Canvas Resource A National Q&A Session will be held during week two (2) of the semester. The Q&A session will unpack the assessment requirements. You will have the opportunity to ask the National LIC any questions or clarifications you require. You are strongly encouraged to attend this session. LOs Assessed LO1, LO2, LO3, LO4, LO5. Task Develop a creative and visually appealing 10-minute narrated educational e-poster based on one of the provided case studies. The e-poster should be targeted towards consumers (not health professionals), much like a poster you would see in a hospital or general practice waiting room. Your assessment should include: • Your name, student number and the case study you have chosen. • The definition and pathophysiology of the chronic condition and disability and how the conditions intersect/relate to one another. • An infographic created by you, that provides some epidemiological statistics. • The factors that impact on quality of life for the case study and the impacts the chronic condition and disability have on quality of life, independence, and activities of living. • An innovative and relevant strategy/action/behaviour change that the person in the case study themselves can implement at home to support their independence and quality of life. Outline the relevance of the identified strategy/action/behaviour change for the person in the case study, and what your (the RNs) role will be in assisting the person to be able to independently complete/achieve the strategy/action/behaviour change. • A drawing/image of a health behaviour theory (for example, the health belief model, theory of planned behaviour etc.) that incorporates considerations from the case study into the model (e.g. think of the case studies age/stage of life, NRSG372: Chronic Illness and Disability NRSG372 Assessment 1: Health Education/Promotion Resource 2024 – © Australian Catholic University Page 2 of 6 their conditions and associated risk factors/symptoms). Indicate how you, as the RN, could apply the theory to encourage the case study to implement the innovative and relevant strategy/action/behaviour change you have identified. • An appropriate and real service/support/group in your local area that could assist the person in the case study to improve their independence and quality of life. Include a description of the real service/support/group in your local area, its relevance and why/how the service will assist the person in the case study. • The role of the NDIS in the case study’s ongoing care needs and how you, as an RN, can promote one of the seven elements of the NDIS Code of Conduct when supporting the person in the case study in the management of their conditions. • Intext referencing must be included on the e-poster. • A reference list must be included on the e-poster. The poster should be visually appealing, with the key points included on the poster, supported by diagrams, figures and imagery. Use your narration to expand on the information presented visually on your poster. Case Study 1 A middle-aged adult with type 2 diabetes and an above knee amputation that occurred five years ago as a result of diabetes-related foot disease. Case Study 2 A young adult with epilepsy (generalised seizures) and autism spectrum disorder. Submission The assessment is to be submitted via the ‘Assignment’ tab of the NRSG372 Canvas unit. INSTRUCTIONS FOR PREPARING, RECORDING & SUBMITTING YOUR ASSESSMENT There are five steps involved in completing your assignment. It is vital you complete all steps and check your submission for the presence of an audio and visual component. If you do not correctly submit to the Canvas dropbox, and your work cannot be viewed or is inaudible, and you will receive a mark of zero. Steps 1. Create your e-poster. 2. Create your e-poster video. 3. Upload your e-poster to the assessment drop box. 4. Upload your e-poster video to the assessment drop box. 5. Check your submission in the assessment drop box and submit. Step 2: Create your video with narration • The e-poster narration is to be 10 minutes long +/- 10%; those exceeding 11 minutes in length will not be listened to after this point. Do NOT speed up the audio component of your recording to fit within the allocated time. • You do NOT need to discuss your reference list in your recorded narration; however, your intext references and your reference list need to be included on your e-poster. • You can choose to create your video using several programs including: o narrated PowerPoint (watch until 1:10); o Echo360 Universal Capture (download the program, then watch this short video for instructions on recording via Echo360), or o Zoom (instructions for zoom) Save your video as a VIDEO file (e.g. .mp4 format).
